Lauren Melvin says those are some of the big problems in this week's 2 wants to Know Restaurant Report Card.

With summer vacation winding down, Lauren Melvin is helping you get out and enjoy it, with 2 Wants to Know Restaurant Report Card.

WFMY -- From the east to the west, we have you covered. Here are this weeks' restaurant scores from across the Triad.

FORSYTH COUNTY:

MIZU JAPANESE CUISINE
3374 ROBINHOOD RD
WINSTON SALEM

The inspector said kitchen equipment, like the meat grinder, and knives had food debris on them. And 75 percent of the food items that require date marks, didn't have them!

SARKU JAPAN
3320 SILAS CREEK PKWY
WINSTON SALEM
90

According to the inspection report, cut cabbage, carrots, and broccoli weren't being stored at a safe temperature. The veggies were thrown out during the inspection. And there was a severe condensation leak in the walk-in freezer and other damage, so it wasn't working properly. They were given 10 days to repair it.

SUSHI THAI

The inspector said food items like cooked chicken and rice were missing date marks. That was corrected during the inspection. But detailed cleaning was needed in the walk-in cooler and freezer, and under the prep tables.
